About the Role
- As a Home Health Support Professional, you will work closely with our interdisciplinary team to develop and implement care plans that meet the unique needs of each client.
- You will provide hands-on personal care, report changes in the client's condition, and perform simple procedures as an extension of therapy or nursing services.
- Additionally, you will assist with incidental/instrumental activities of daily living, such as bathing, dressing, grooming, light housekeeping, meal preparation, shopping, transportation, and companionship.
Responsibilities
- Provide one-on-one care and support to clients, assisting with activities of daily living
- Help clients with ambulation, transferring, and range of motion exercises
- Provide medication reminders and document the client's condition
- Monitor and report changes in the client's condition to the healthcare team
Qualifications
- 6 months prior adult caregiving experience OR completion of a CNA course/certificate
- 2 positive references from previous employers or professional organizations
