Caregiver/Home Health Aide (HHA)
We are seeking individuals to join our interdisciplinary team who provide hands-on personal care, report changes in the client's condition, perform simple procedures as an extension of therapy or nursing services, and provide incidental/instrumental activities of daily living to clients in the home setting.

Job duties include:
- Providing personal care and support seniors need to live safely at home
- Assisting with activities of daily living such as bathing, dressing, grooming, light housekeeping, meal preparation, shopping, transportation and companionship
- Helping with ambulation, transferring and range of motion exercises
- Providing medication reminders, documenting their condition and notifying a supervisor of any concerns
- Maintaining a safe home environment with unobstructed pathways
- Participating in activities that bring clients joy such as puzzles, games, reading and hobbies
Requirements for this role:
- 6 months prior adult caregiving experience OR completion of a CNA course/certificate
- 2 positive references
