Personal Care Aide

**Job Title:** Home Health Aide (HHA) / Caregiver

We are seeking a dedicated and compassionate Home Health Aide (HHA) to join our interdisciplinary team. As a member of our team, you will provide hands-on personal care, report changes in the client's condition, perform simple procedures as an extension of therapy or nursing services, and provide incidental/instrumental activities of daily living to clients in the home setting.

Responsibilities:

• Provide personalized care and support to seniors, including bathing, dressing, grooming, light housekeeping, meal preparation, shopping, transportation, and companionship
• Assist with ambulation, transferring, and range of motion exercises to help clients maintain their mobility and independence
• Document the client's condition and notify a supervisor of any concerns or changes in their status
• Ensure a safe home environment by maintaining unobstructed pathways and performing other safety-related tasks as needed

Requirements:

• 6 months of prior adult caregiving experience or completion of a Certified Nursing Assistant (CNA) course/certificate
• Two positive professional references
