**Responsibilities**
- Assess the health status of residents and develop personalized care plans.
- Collaborate with healthcare professionals, including physicians and therapists, to ensure seamless care transitions.
- Provide education and support to residents and their families on disease management, preventative care, and healthy lifestyle choices.
- Supervise CNAs and PCAs on your shift, ensuring that residents receive high-quality care.
- Maintain accurate medical records and document all assessments, interventions, and communications related to resident care.
**Requirements**
- RN degree from an accredited school of nursing.
- Current and valid RN license in the state of Minnesota.
- Previous experience in geriatric nursing.
- Excellent verbal and written communication skills.
- A compassionate and empathetic approach to working with senior residents.
- Strong critical thinking and problem-solving skills.
