About the Role:
This is an exciting opportunity for a skilled RN to join our team as we provide comprehensive care services to assisted living and memory care communities throughout the Twin Cities metro area. As a member of our team, you'll work closely with other healthcare professionals to deliver high-quality care, promote healthy lifestyles, and foster open communication with residents, families, and staff.
Responsibilities:
- Conduct comprehensive health assessments, medication management, and overall health status monitoring for residents in assisted living and memory care facilities.
- Supervise CNA's/PCA's on your shift to ensure seamless care delivery.
- Provide health education to residents and their families on various topics, including disease management, preventative care, and healthy lifestyle choices.
- Coordinate and communicate with other healthcare professionals to ensure comprehensive and holistic care for residents.
- Respond promptly and efficiently to medical emergencies, following established protocols and providing immediate care as needed.
- Maintain accurate and up-to-date medical records, documenting all assessments, interventions, and communications related to resident care.
- Collaborate with the Director of Nursing, nursing staff, and other team members to maintain a cohesive and supportive care environment.
- Foster open and transparent communication with residents, their families, and staff to promote a positive and collaborative care environment.
