About the Role
This is an exceptional opportunity for an experienced geriatric nurse to make a meaningful difference in the lives of our senior residents. As a RN Care Manager, you will be responsible for conducting comprehensive health assessments, managing medications, and monitoring the overall health status of our residents. You will also supervise CNAs/PCAs on your shift, coordinate with other healthcare professionals, and communicate effectively with residents, families, and staff.
Responsibilities
- Case Management: Conduct comprehensive health assessments, manage medications, and monitor the overall health status of residents.
- Care Coordination: Supervise CNAs/PCAs on your shift, coordinate with other healthcare professionals, and communicate effectively with residents, families, and staff.
- Health Education: Provide education to residents and their families on disease management, preventative care, and healthy lifestyle choices.
- Emergency Response: Respond promptly and efficiently to medical emergencies, following established protocols and providing immediate care as needed.
Benefits
