Responsibilities:
A Home Health Admission RN will be responsible for conducting home visits, assessing the patient's condition, progress toward goals, providing patient care, teaching, and other skilled interventions as ordered. They will also complete initial and ongoing comprehensive assessments, coordinate patient/client services, and participate in clarifying mutually agreed upon goals.
Qualifications:
- Graduate of an accredited nursing program and active RN license in Colorado
- Minimum of one (1) year of nursing experience, ideally in home healthcare
- OASIS experience preferred
- CPR Certification required
- Pass a background check and drug test
- Able to work independently and provide effective communication
- Knowledge of state, federal, and licensure regulations
