Job Summary
We are seeking an experienced Home Health Admission Registered Nurse to join our team. The ideal candidate will have a strong background in nursing, preferably in home healthcare, and excellent communication skills. This role requires a collaborative approach to patient care, working closely with Case Managers and other healthcare professionals to develop care plans that meet the unique needs of each patient.
Responsibilities
- Complete initial and ongoing comprehensive assessments
- Coordinate patient/client services
- Provide care according to the patient's/client's plan of care/service plan
- Participate with the patient/client in clarifying mutually agreed upon goals and incorporate these goals into the plan of care/service plan
- Conduct home visits as assigned, assessing the patient's condition, progress toward goals, providing patient care, teaching and other skilled interventions as ordered
Requirements
- Graduate of an accredited nursing program and active RN license in Colorado
- Minimum of one (1) year of nursing experience, ideally in home healthcare
- OASIS experience, preferred
- CPR Certification (demonstration course required)
- Pass a background check and drug test (we are required to test for THC)
- Able to work independently and provide effective communication to patient/family, team members, and other health care professionals
- Knowledge of state, federal, and licensure regulations
