\As a Home Health Physical Therapist, you will play a vital role in our team, providing critical care to patients in the comfort of their own homes.
\You will conduct thorough assessments, develop personalized care plans, and work collaboratively with our interdisciplinary team to ensure seamless care transitions.

\Requirements
\To succeed in this role, you must be a graduate of an accredited Physical Therapist Program and hold an active PT license in Colorado. A minimum of one year of physical therapy experience, preferably in home healthcare, is also required.
\You should possess good clinical judgment, strong interpersonal skills, dexterity, and compassion. A BLS/CPR certification is also necessary, with a demonstration course required.
