Job Summary
We are seeking a skilled Home Health Physical Therapist to join our team at Interim HealthCare. As a Home Health Physical Therapist, you will work closely with patients in their homes, providing personalized therapy to help them regain mobility, strength, and functionality.
About the Role
- Provide home-based physical therapy services to patients who have experienced illness or injury.
- Work collaboratively with a multidisciplinary team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to develop and implement patient care plans.
- Conduct thorough assessments, observe patient movements, and document progress to inform treatment decisions.
- Design and deliver customized exercise programs, provide hands-on therapy, and educate patients on self-care techniques to manage pain and improve mobility.
- Counsel patients and their families on established goals, exercises, and safe practices to support ongoing recovery.
Requirements
- Graduate of an accredited Physical Therapist Program and active PT license in Colorado.
- A minimum of 1 year of experience in physical therapy, preferably in home healthcare.
- BLS/CPR certification (demonstration course required).
- Knowledge of state and federal regulations governing home health care.
- Excellent clinical judgment, strong interpersonal skills, dexterity, and compassion.
- Possess good communication skills to effectively collaborate with patients, families, and healthcare teams.
