Job Description
As a Home Health Physical Therapist, you will provide home-based therapy to patients who have lost mobility, strength, and functionality due to an illness or injury. You will work as part of a home health team focused on the patient's plan of care and goals. Your responsibilities will include observing movements, conducting assessments, documenting progress, and modifying care plans as needed.
Required Skills and Qualifications
- Graduate of an accredited Physical Therapist Program
- Active PT license in Colorado
- Minimum 1 year of physical therapy experience, ideally in home healthcare
- BLS/CPR certification
- Knowledge of state and federal home health regulations
