Job Description
">- As a Home Health Physical Therapist, you will provide personalized, home-based therapy to patients who have lost mobility, strength, and functionality due to an illness or injury.
- You will work as part of a home health team focused on the patient's plan of care and goals, which may include an RN, LPN, CNA, HHA, OT, and SLP.
- Your responsibilities will include observing movements, conducting assessments, documenting progress, and modifying care plans as needed.
- You will assist patients with exercises and hands-on therapy to manage pain and increase mobility.
- Educate patients on their plan of care, established goals, exercises, and self-care.
- Coach family members on the plan of care, therapy goals, and safely assisting with exercises.
Required Skills and Qualifications
">- A graduate of an accredited Physical Therapist Program and an active PT license in Colorado are required.
- Minimum 1 year of physical therapy experience, ideally in home healthcare, is preferred.
- BLS/CPR certification (demonstration course required) is necessary.
- Knowledge of state and federal home health regulations is essential.
- Good clinical judgment, strong interpersonal skills, dexterity, and compassion are essential qualities for this role.
- A background check and drug test are required, including testing for THC.
