Responsibilities
- Provide home-based physical therapy services to patients with a focus on improving mobility, strength, and functionality.
- Collaborate with the home healthcare team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to develop and implement patient care plans.
- Conduct assessments, observe patient movements, and document progress to modify care plans as needed.
- Educate patients on their plan of care, established goals, exercises, and self-care techniques.
- Clients include adults requiring rehabilitation following surgery, illness, or injury. Assessing the need for durable medical equipment and supplies for optimal recovery and independence.
Requirements
- Graduate of an accredited physical therapist program and active PT license in Colorado.
- A minimum of one year of physical therapy experience, preferably in home healthcare.
- BLS/CPR certification is required, with a demonstration course provided.
- Knowledge of state and federal home healthcare regulations.
- Excellent clinical judgment, strong interpersonal skills, dexterity, and compassion are essential.
- A background check and drug test will be administered as part of the hiring process.
