Job Description
We are seeking a home health physical therapist to provide patient-centered care in the comfort of their own homes. As part of our interdisciplinary team, you will work closely with patients, families, and other healthcare professionals to develop and implement personalized plans of care.
- Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to an illness or injury.
- Work as part of a home health team focused on the patient's plan of care and goals.
- Observe movements, conduct assessments, document progress, and modify care plans as needed.
- Assist patients with exercises and hands-on therapy to manage pain and increase mobility.
- Educate patients on their plan of care, established goals, exercises, and self-care.
- Couch family members on the plan of care, therapy goals, and safely assisting with exercises.
Required Skills and Qualifications
To be successful in this role, you must have:
- A graduate degree from an accredited physical therapist program and active PT license in Colorado.
- A minimum of 1 year of physical therapy experience, ideally in home healthcare.
- BLS/CPR certification (demonstration course required).
- Knowledge of state and federal home health regulations.
- Good clinical judgment, strong interpersonal skills, dexterity, and compassion.
- The ability to pass a background check and drug test.
