We are seeking a highly skilled Physical Therapist (PT) to join our team as a Home Health PT in Pueblo, CO.
About the Job
- This is a Full-Time and Part-Time opportunity for a Physical Therapist to provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to an illness or injury.
- As a Home Health PT, you will work as part of a home health team which may include an RN, LPN, CNA, HHA, OT, COTA, PTA, and SLP, focused on the patient's plan of care and goals.
Your Responsibilities
- Provide one-on-one physical therapy to patients in their homes, assisting them with exercises and hands-on therapy to manage pain and increase mobility.
- Educate patients and their families on their plan of care, therapy goals, home exercises, and self-care.
- Observe movements, conduct assessments, document progress, and modify the care plan as needed.
Requirements
- A graduate of an accredited Physical Therapist Program and an active PT license in Colorado.
- A minimum of three years of physical therapy experience, ideally one year in home healthcare.
- CPR Certification.
- Good clinical judgment, strong interpersonal skills, excellent time management, and compassion.
