About the Job
This role involves providing home-based physical therapy to patients who have lost mobility, strength, and functionality due to an illness or injury. As a member of a home health team, you will work alongside healthcare professionals such as RNs, LPNs, CNAs, HHAs, OTs, and SLPs focused on delivering patient-centered care and meeting individualized goals. Your key responsibilities include observing movements, conducting assessments, documenting progress, and modifying care plans as needed. Additionally, you will assist patients with exercises and hands-on therapy to manage pain and increase mobility, educate patients on their plan of care, established goals, exercises, and self-care, and coach families on the plan of care, therapy goals, and safely assisting with exercises.
Requirements and Qualifications
To be successful in this role, you must possess a graduate degree from an accredited Physical Therapist Program and hold an active PT license in Colorado. A minimum of one year of experience in physical therapy, preferably in home healthcare, is also required. Furthermore, you should have BLS/CPR certification and knowledge of state and federal home health regulations. Strong clinical judgment, interpersonal skills, dexterity, and compassion are essential for this position. Finally, you will need to pass a background check and drug test.
