General Purpose:
To supplement Palliative Care and provide our patients with care over chronic illness in the comfort of their own homes/the facilities and buildings we help service. The position requires this individual to be self-driven and help continue to build the Wisconsin Rapids program based on the success of our other service areas and their programs. Visits to patients for medical assessment and intervention whether in a facility or the home and completing required Face-to-Face visits.
Essential Functions:
- Provides oversight of the Nurse Practitionerservices to palliative patientsensuring continuity of medical services including appropriate management of patient pain and symptoms related to the chroniccondition.
- Documents an individual patients progression in end-stage disease in the patient/family record as appropriate.
- Actively participates in Stand Upmeetings in the morning.
- Updateeach patients comprehensive assessment and plan of care as often as a patients condition requires and in accordance with applicable law and regulation.
- When needed, participates in:
- Patient continuing care decisions;
- Develop your territory by connecting with patients/families and facilities to garner referrals and add to your patient census.
- Acts as a patients Nurse Practitioner to meet general medical needs, including pain and/or symptom management or other medical intervention should a patients attending physician/Nurse Practitioner not be available or choose to relinquish responsibility for a patients care.
Minimum Education & Experience Requirements:
- An advanced degree in nursingin WI
- Current license in good standing in the state(s) in which the programprovides care.
- Hospice/palliative experience is preferred.
