Qualifications
To be successful in this role, you will need to possess a current unrestricted license to practice as a Registered Nurse (RN) in the state associated with this position. Additionally, you should have one year of professional experience practicing as a Registered Nurse in home health or a similar setting. Previous case management/utilization review experience and OASIS experience are preferred, but not required.

- Conducting in-person patient interviews and comprehensive physical assessments
- Overseeing the implementation and ongoing assessment of the patient's plan of care
- Communicating with patients and their families to deliver education on proper home health care procedures
- Managing a team of home health aides, LPNs, RNs, and other caregivers
