About the Role
- Deliver personalized home-based physical therapy services to patients who require assistance with mobility, strength, and functionality due to illness or injury
- Collaborate with a multidisciplinary team, including nurses, CNAs, HHAs, OTs, and SLPs, to develop and implement patient care plans
- Conduct thorough assessments, document progress, and modify treatment plans as needed
- Educate patients on their plan of care, established goals, exercises, and self-care techniques
- Counsel family members on the plan of care, therapy objectives, and safely assisting with exercises
Key Qualifications
- Graduate of an accredited physical therapist program and active licensure in Colorado
- At least 1 year of experience in physical therapy, preferably in home healthcare
- BLS/CPR certification (demonstration course required)
- Knowledge of state and federal regulations governing home health care
- Excellent clinical judgment, strong interpersonal skills, manual dexterity, and compassion
- Pass a background check and drug test (required for THC)
