**Job Description**
As a Home Health Physical Therapist at Interim HealthCare, you will provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to an illness or injury. You will work as part of a multidisciplinary team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, focused on the patient's plan of care and goals.
You will observe movements, conduct assessments, document progress, and modify the care plan as needed. You will assist patients with exercises and hands-on therapy to manage pain and increase mobility. Additionally, you will educate patients on their plan of care, established goals, exercises, and self-care. You will also coach families on the plan of care, therapy goals, and safely assisting with exercises.
