Job Description:
We are seeking a skilled and compassionate Physical Therapist to join our team at Interim HealthCare. As a Home Health Physical Therapist, you will provide personalized care to patients in their own homes, helping them regain mobility and independence.
In this rewarding role, you will work closely with patients, family members, and other healthcare professionals to develop and implement tailored treatment plans. You will observe patient movements, conduct assessments, and document progress, modifying care plans as needed to ensure optimal outcomes.
You will also educate patients on their plan of care, established goals, exercises, and self-care techniques, empowering them to take control of their health. Additionally, you will coach family members on the plan of care, therapy goals, and safely assisting with exercises, promoting a supportive environment for patients' recovery.
Requirements:
- A graduate degree from an accredited Physical Therapist Program
- An active PT license in Colorado
- At least 1 year of experience in physical therapy, preferably in home healthcare
- BLS/CPR certification (demonstration course required)
