What are the responsibilities and job description for the Social Worker position at Iowa County Corporation?
JOB REQUIREMENTS : Provide ongoing services to the clientele assigned to the position including case management, conducting initial assessments and maintaining required documentation of all activities for the Iowa County Department of Social Services. -Perform all duties of intakes and / or admission to programs. -Comply with the state statutes that govern the assigned worker\'s area of practice. -Complete assessments through interview, observations, and the use of other appropriate techniques. -Maintain client confidentiality. -Learn the appropriate database system and document information as is required to maintain reporting practices. -Identify safety issues. -Take steps to ameliorate any identified safety issues. -Provide ongoing case management services including the development a case plan, implementation and ongoing monitoring of a case plan for providing services to meet the needs of the client. -Complete required documentation within the established guidelines -Locate appropriate substitute or alternate care settings. -Develop and submit appropriate reports to the court and provide testimony if necessary. -Act as back-up in other service areas as needed. -Make referrals and monitor ongoing services. -Complete required documentation and other paperwork within established guidelines and required timeframes. -Provide presentations and / or training to other staff, clients, and the community as required. -Complete investigations to determine if abuse and / or neglect has occurred. -Be available for case consultation and other issues after normal working hours. - Maintains a positive and helpful working relationship with other professionals. -Participate in regular inter-disciplinary team meetings as required. -Complete written reviews and update case plans and goals as necessary. -Contributes proactively to staff team building and effectiveness. -Performs coordination of services needed for case closures or other discharge. -Act as an advocate for the client and promote their rights as an individual and / or resident. -Works with various Human Services agencies to assist with the financial, social, and legal needs of the client / resident. -Monitor resident / client behavior and implement interventions as required. -Promptly coordinate and summarizes investigations. -Attends and participates in meetings as required. -Attend appropriate training in order to be compliant with standards. -Must demonstrate regular and dependable attendance. -Obtain signatures from residents / clients / guardians for case plans, release of information forms, and medical consent forms. -Recruit, train, and monitor volunteers to the agency. -Perform other duties as assigned.
- OTHER EXPERIENCE AND QUALIFICATIONS : Minimum requirement of Bachelor\'s degree in Social Work or other closely related field Preference for one year recent professional experience in the related area of which you are applying, or any combination of education and experience that provides equivalent knowledge, skills, and abilities Minimum requirement of current Wisconsin Social Work certification or the ability to attain the certification within two years of employment.
