What are the responsibilities and job description for the Care Manager (Qualified Professional) position at IQuOLIOC, Inc.?
Due to business growth and demand, IQuOLIOC is seeking a Care Manager to work under the direct supervision of the Clinical Director. A Care Manager will manage and coordinate services for program participants.
Responsibilities and Duties:
Assesses client needs and the development of the support plan specific to the assessment outcomes
Manages a caseload of substantial size and complexity requiring the utilization of all community agencies serving persons with developmental disabilities
Responsible for participant caseload, including, but not limited to annual documents, Short Range Goals, Personal Care Plans, NC SNAPs, FL-2s, and tracking of document and assessment expiration
Prepares case progress notes, social summaries, and other reports concerning the participant's case
Provides staff training, remedial training, staff mentoring, and enrichment
Supervises a variety of participant care duties involving management and care of participants; assist in evaluation of staff and program needs
Participates in implementing clinical care practices; provides input on issues
Participates in multi-disciplinary treatment planning
Ensures a positive work environment
Other duties as assigned
Qualifications and Skills
Essential Skills Required:
Knowledge of the principles of supervision
Excellent command over the language, both written and verbal
Work cooperatively with others
Educational/Professional Qualifications:
Master’s degree in human service field and a minimum of one year full-time, post-graduate degree accumulated IDD experience with the population served preferred. Bachelor's degree in a human service field and two years of full-time, post-bachelor's degree accumulated IDD experience with the population served or bachelor's degree in a field other than human services and four years of full-time, post-bachelor's degree accumulated IDD experience with the population served minimally required.

Pay: $16.44 - $21.41 per hour
