Title: Family Physician Assistant or Nurse Practitioner
FLSA Status: Salary – Exempt/ Full-time

Job Summary: ICH is a team-based primary care center with integrated onsite behavioral health, psychiatry, RN care management, laboratory, and pharmacy services at our main location in Mt. Pleasant. The Farwell Health Center has onsite laboratory and pharmacy. The PA or NP will provide direct primary care services to all ages in a clinical setting under direct supervision of a physician. The PA or NP must assure accurate and timely completion of charting in the patient medical record. The PA or NP will work collaboratively to create and maintain a culture of dedication to providing compassionate, high-quality health care to the community. ICH follows the Patient Centered Medical Home modal of care with this designation from the National Committee for Quality Assurance. This practice is a Monday-Friday outpatient-only practice.
Qualifications:
Required
- Graduate of an accredited Physician Assistant or Nurse Practitioner training program
- Licensure as a Physician Assistant or Nurse Practitioner in Michigan
- Board certified
- Active and unrestricted Drug Enforcement Administration (DEA) license
- BLS certification
Preferred
- 3 years of primary care outpatient clinic experience
