Job Summary
We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care, collaborating with healthcare professionals, and ensuring optimal outcomes for patients.
Duties
- Conduct patient assessments and develop individualized care plans
- Coordinate and communicate with healthcare providers to ensure comprehensive patient care
- Manage medical records and ensure accuracy and confidentiality
- Provide direct patient care in a Level I Trauma Center setting
- Utilize physiology knowledge to assess and monitor patient conditions
- Implement case management strategies to optimize patient outcomes
Skills
- Strong background in ICU or hospital medicine
- Proficient in patient assessment and care planning
- Experience working in a Level II Trauma Center is a plus
- Excellent communication and collaboration skills
- Ability to manage complex cases efficiently
- Knowledge of case management principles and practices
Job Type: Contract
Pay: $47.00 - $50.00 per hour
Expected hours: 40 per week
Schedule:
- 10 hour shift
- 8 hour shift
- Day shift
- Monday to Friday
Willingness to travel:
- 25% (Preferred)
