J. C. Lewis Primary Health Care Center is a federally qualified community health center which exists to provide patient-centered primary health care services to the medically underserved residents of the greater Savannah-Chatham County, Georgia community. The Health Center makes comprehensive primary care, integrated behavioral health care, oral health care, and inpatient respite care services available to all persons, including the homeless and low-income uninsured and underinsured, regardless of ability to pay. Position Overview
The Community Outreach Case Manager will be responsible for providing case management services to individuals and families in need, primarily focusing on linking clients to resources, supporting them through challenges, and advocating for their needs in the community. This role involves working closely with local organizations, service providers, and community members to improve access to services and promote the well-being of underserved populations.
Key Responsibilities
- Assess clients’ needs through intake assessments, interviews, and evaluations.
- Provide one-on-one case management support to clients, including guidance, referrals, and coordination with other community services.
- Track client progress and maintain case records, ensuring all documentation is up to date, accurate, and confidential.
- Organize and participate in outreach activities, events, and informational sessions to raise awareness of available services.
- Build relationships with local organizations, agencies, and stakeholders to facilitate client access to resources.
- Identify and connect clients with appropriate community resources, including housing, food assistance, healthcare, and mental health services
- Maintain a comprehensive list of community resources and ensure clients receive timely and effective support.
- Track and report on client outcomes, services provided, and challenges encountered.
- Prepare and submit reports as required by program managers
Qualifications
- Associates degree in Social Work, Psychology, Sociology, or a related field preferred
- Equivalent combination of education and experience may be considered.
- Minimum of 1 year of experience in case management, social services, or community outreach work preferred.
- Experience working with diverse populations, including low-income individuals, individuals experiencing homelessness, or those with mental health or substance abuse challenges
Skills and Abilities
- Strong knowledge of community resources, social service programs, and local agencies.
- Excellent communication, interpersonal, and organizational skills.
- Ability to manage a caseload, prioritize tasks, and meet deadlines.
- Empathy and sensitivity to the needs and challenges faced by vulnerable populations.
- Ability to work independently as well as part of a team.
- Bilingual (English/Spanish ) a plus.

Pay: $18.25 per hour
