As an Occupational Therapist, you will play a vital role in helping patients recover from injuries and illnesses, and regain their independence. Your primary responsibility will be to assess patients' strengths and weaknesses, and develop individualized treatment plans to help them achieve their goals.
In addition to providing high-quality patient care, you will also be responsible for maintaining accurate records of patient care data, including initial evaluations, daily notes, reassessments/progress reports, discharge plans, and communication with members of the team. You will also be expected to prepare for evaluations, select and administer tests and measures, and synthesize evaluation results to formulate diagnoses, goals, prognosis, and plans of care.
Responsibilities:
- Assesses patients' strengths and weaknesses to devise effective treatment plans.
- Develops and implements treatment plans that cater to the unique needs of each patient.
- Collaborates with interdisciplinary team, patients and patient's support system to ensure seamless care delivery.
- Maintains accurate records of patient care data, including initial evaluations, daily notes, reassessments/progress reports, discharge plans, and communication with members of the team.
- Selects and administers tests and measures to gather relevant data.
Requirements:
- Bachelors or Masters Degree required, Doctorate degree preferred
- Licensed Occupational Therapist
- Completed state regulated CEU requirement in appropriate state (NY, NJ)
- Valid CPR License
- Experience/knowledge with an EMR software is preferred
- Excellent oral, written communication
- Strong clinical decision-making skills
- 1 year(s) professional OT experience
- Excellent work ethic and dedication to patient success
- Basic computer skills
- Works as a team member and individually with minimal supervision
- Maintains a positive, professional demeanor at all times
