Overview We are looking for a Nurse to join our Home Health team. In this role, you will provide high-quality nursing care to patients in the comfort of their own homes. Duties
- Conduct comprehensive patient assessments to determine care needs and develop individualized care plans.
- Administer medications and treatments as prescribed, including tube feeding and venipuncture.
- Monitor vital signs and perform triage to assess patient conditions effectively.
- Provide education and support to patients and their families regarding health management and disease prevention.
- Collaborate with interdisciplinary teams to coordinate care for patients with complex health issues, including infectious disease care and acute care needs.
- Document patient progress and maintain accurate medical records in compliance with healthcare regulations.
- Assist in cardiac catheterization procedures as required, ensuring patient safety throughout the process.
- Support patients undergoing dialysis by providing necessary care and monitoring.
Skills
- Strong skills in patient assessment, critical thinking, and clinical decision-making.
- Experience with tube feeding techniques and managing complications associated with it.
- Ability to perform venipuncture competently and safely.
- Knowledge of acute care practices and protocols for effective patient management.
- Familiarity with cardiac catheterization procedures is a plus.
- Understanding of infectious disease management principles.
- Excellent communication skills to educate patients effectively about their health conditions.
