JAMAICA HOSPITAL MEDICAL CENTER, a major health care facility located in Queens, N.Y. is seeking an Care Management Nurse for the Population Health/Ambulatory Care department.

Job Summary:
- The Care Transition Nurse plays a critical role in ensuring seamless transitions for patients moving between different levels of healthcare settings, such as from hospitals to ambulatory clinics to home care or rehabilitation facilities. This position includes medication reconciliation post-discharge from hospital, follow-up after emergency department (ED) visits for multiple chronic conditions, and follow-up after hospital discharges.
- The Care Transition Nurse will work with the high risk, high cost and complex patient cohorts to better manage their chronic conditions, medications, health-related social needs, referrals, and all care transition-related activities
Job Responsibilities:
- Management of Complex patients with multiple chronic conditions:
- Will be part of the care team in managing multiple chronic conditions via care plans developed by the patients’ PCP.
- Outreach and monitoring of condition, medication management between visits to optimize medical care.
Follow-Up After ED Visits:
- Conduct follow-up calls and visits for patients with multiple chronic conditions to ensure they receive appropriate care and avoid readmissions.
- Assess patient needs and coordinate necessary services, such as home health care or specialist referrals.
- Monitor patient progress and adjust care plans as needed.
Follow-Up After Hospital Discharges:
- Coordinate with healthcare providers to ensure continuity of care and monitor patient progress post-discharge.
- Develop individualized care plans that address patient-specific needs and goals.
- Facilitate timely follow-up appointments with primary care providers and specialists.
Patient Education:
- Educate patients and their families on post-discharge care instructions, medication management, and the importance of follow-up appointments.
Required Skills/Experience:
- Experience: Minimum of 2 years of clinical nursing experience, preferably in a hospital or managed care setting.
- Skills: Strong clinical assessment skills, excellent communication and interpersonal skills, ability to work collaboratively with a multidisciplinary team, and proficiency in electronic health records (EHR) systems.
- Certifications: Certification in Care Coordination and Transition Management (CCTM) is preferred.
- Proficiency in PC and EMR documentation.
- Excellent communication skills, both written and verbal
Qualifications:
- Valid RN license in New York State.
- BSN required
- Previous experience in care management or related field preferred.
- Strong organizational and communication skills.
- Ability to work collaboratively in a fast-paced environment.
- Compassionate and patient-centered approach to care.
- Prior hospital experience a plus
