Description
Intake Specialist Responsibilities include the following. Other duties may be assigned.
- Receives and reviews referrals, contacts and evaluates consumer’s eligibility for enrollment, determines what funding source the consumer participates in as outlined by program requirements.
- Provides full explanation of the Article 16 program, following appropriate guidelines and protocols to consumers and/or families.
- Verifies insurance and demographic information.
- Provides patient with pre-appointment information and/or education appropriate to the nature of the visit.
- Obtains insurance authorizations for services as required by insurance company.
- Prepares consumer intake charts for treatment planning by therapist. Obtains all required documentation for consumers to participate in Article 16 program and forwards forms for approval.
- Contacts therapist when the new patient packet and prescription is completed.
- Assists the therapists with scheduling appointments.
- Prepares and maintain patient charts (including filing and scanning).
- Ensure reports and records from outside organizations are secured for internal use and record keeping for Article 16 consumers.
- Monitors requirements for new script and notifies practitioner.
- Forwards written treatment plan to the Service Coordinators, group homes, primary physicians and Article 16 Medical director, and as appropriate to the caregivers and other referral sources.
- Interacts and communicates with physician offices, insurance companies and consumers for the purpose of obtaining information regarding the patient and services provided.
- Performs confirmation calls as needed.
- Copies/scans documents for patients requiring medical records.
- Answers calls and schedules appointments as required.
- Sends out information packets for new patient visits.
- Assists with calls to individuals’ families regarding insurance issues
- Updates patient demographic information in various Jawonio systems.
- Ensures patient paperwork and surveys are distributed as needed.
- Assists Program Director with monitoring and upkeep of the clinic physical space.

Qualifications
Education/ Experience:
Prefer candidate to have 2 years of experience in a similar role in a clinical setting and working with OPWDD population. Candidate must possess proficient computer skills including MS Office (Outlook, Excel, Word, and PowerPoint).
Additional Requirements:
- Remain current in all required trainings.
- Must be cleared and maintain acceptable record under the NYS mandated criminal background check process.
- Must be clear and maintain an acceptable record under the Medical Fraud &Abuse clearance process.
