POSITION:
• Habilitation Services Supervisor
GENERAL DESCRIPTION:
• To develop, implement, and supervise Habilitation Services in collaboration with the organization’s Employment Team, Regional Centers, and Department of Rehabilitation.
ESSENTIAL RESPONSIBILITIES/DUTIES: Essential functions of the job are denoted. Other duties may also be assigned. Assure quality services for the individuals served by the program:
• Conduct intake and orientation procedures for all individuals who are referred to Habilitation Services. These procedures will include: conducting the initial intake meeting, assessment and evaluation, arranging and facilitating the person’s intake with the funding source (Regional Center, Department of Rehabilitation, or other.)
• Develop, implement, and monitor the IHSP and supports for each individual using specific and measurable goals or Smart Goals based on the individual’s employment goals.
• Arrange for and oversee services and supports to implement and monitor individuals’ services, including:
• Schedules of services and supports for individuals.
• All pre-placement, job support, and follow-up services.
• Spending time with the individuals and Job Coaches in the community
• Track, manage, and ensure quality standards and compliance for all required tracking reports for each individual’s progress and support needs.
• Identify issues and strategies to assure quality services for each individual and the service as a whole.
• To work with the Employment Services Representative(s), Job Developers and Employment Specialist, regarding on the job support; specifically around training social/behavioral skills related to work, and specific job/task analysis, teaching strategies, and/or accommodation as needed on the job
• Initiate and participate in circle meetings for individuals in the program.

Assure an effective workforce in order to provide high quality services:
• Recruit, screen, hire and train Job Coaches to meet the needs of each individual in the services, including the individual served in the process.
• Supervise, train, direct, and evaluate the services and Job Coaches; orient them to Jay Nolan Community Services (JNCS) policies and procedures, the Initial Training Program (ITP), and Employment Specialist Training Course and other training needed for their jobs.
• Provide support and training for staff on a daily basis for quality services and maximum community participation of each individual.
• Conduct periodic evaluation of staff performance, including the following;
• Conduct supervisions with staff on a 1:1 basis as frequently as needed but no less than once per quarter.
• Complete annual performance evaluations for each staff as required.
Develop and maintain effective relationships and resources to support the needs of the individuals served:
• Maintain effective relationships with the funding sources and other stakeholders.
• Familiarize and abide with all the organization’s policies and procedures.
• Maintain professional conduct at all times including respectful language, appropriate dress and hygiene.
• Participate and complete the Initial Training Program (ITP) within 90 days of employment.
• Participate in regular work supervision and other training developments/opportunities as required/identified with position.
• Strive for excellence through professional growth, which may include reading relevant current literature, taking courses and attending conferences pertaining to job responsibilities.
• Complete all necessary administration specified in managing service – mileage sheets/ payroll/ SIRS/cyber security training in a timely matter.
• Confirm that Special Incidents are reported accurately and in a timely manner as required including:
• Incident is described appropriately in detail.
• Action taken is consistent with the description of the incident.
• Corrective action planning is recorded and implemented.
• Report is submitted in a timely manner to meet requirements of the agency and the funders.
• Practice effective communication by responding to emails, phone calls, and other communication in a timely manner (1-2 business days).
• Lead fiscal management of the services by the following:
• Develop services in alignment with funding and authorizations.

Manage scheduling and payroll including overtime to support responsible
• Work in partnership with the Finance Department to support responsible fiscal management.
• Complete all financial documentation, tracking and reporting accurately and in a timely manner
Responsible for effective administrative management of the program:
• Be responsible for submitting progress reports, billing records and other documentation to funding agencies as required accurately and in a timely manner.
• Use information tracking systems within the agency effectively
• Participate in management team meetings and contribute to efforts to improve the quality of services provided.
• Contribute to the agency’s Strategic Plan as assigned.
• Monitor the accuracy of all program and individual files and records.
• Maintain all CARF standards, including the program evaluation system and participate in CARF accreditation reviews.
• Meet with the Director/Managers on a regular basis and attend all staff team meetings.
MINIMUM QUALIFICATIONS: The following are the minimum qualifications which an individual needs in order to successfully perform the duties and responsibilities of this position. Please note that the minimum qualifications may vary based upon the department size and/or geographic location.
• Bachelor’s Degree, or equivalent professional qualification plus three years relevant experience in vocational training and/or supported employment for individuals with disabilities.
• Two years of experience in a supervisor/management position preferred.

PHYSICAL DEMANDS: In general, the following physical demands are representative of those that must be met by an employee to successfully perform the essential functions of this job. • Must be able to see, hear, speak and write clearly in order to communicate with employees and/or other customers; manual dexterity required for occasional reaching and lifting of small objects, and operating office equipment.
• Travels as required meeting functions of job responsibilities.
• This position may require the use of smart phones and other mobile electronic devices to perform work functions. The employee’s use of company-issued devices or pre-authorized use of the employee’s personal device will comply with the agency’s Cell Phone Policy at all times.
