RESPONSIBILITIES
SUMMARY: The Homebound Instruction Program is designed to serve students who are unable to attend school for a specifically defined period of time because of a physical and/or mental health concern. This unique program is available to students who are enrolled in a Jefferson County Public School. This program is not a component of Home Based, or Home School Programs. As a district-wide support, the Medical Homebound Instruction Program is not a stand-alone program and relies upon the direction and support of the home school to provide instructional materials, assignments, and all tests. While Medical Homebound instruction cannot replace the classroom experience, unique supports are developed to provide educational support for each student. Parents, home school staff and Medical Homebound Instruction Teachers and staff all work consistently toward positive collaboration for the benefit and success of the student.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Provide direct instruction to students in a home setting, online setting, or in a public setting. An adult 21 years of age or older must be present during the instruction.
Work collaboratively with school-based staff to plan and modify curriculum and to communicate the abilities and needs of students with IEPs and 504 plans.
Ensure and assist the student with maintaining educational continuity during the time they are unable to regularly attend their home school.
Medical Homebound Teachers voluntarily accept assignments. Student assignments are based upon student needs. Ongoing or full time work is not guaranteed.
Once a teacher accepts the student assignment, Medical homebound teachers are expected to follow district protocols and policies.
Support student instruction in the home setting as directed through the educational planning of the home school.
Serves as a liaison and promote communication between the student, school, and parents so that the student continues to be included as a member of the school community.
The Medical Homebound Instructor will support individualized educational planning discussions and problem solving as necessary with students, parents, school teams, administrators to implement service that supports each student's unique needs, including the needs specified in a student's 504 plan.
Support the student's transition back to the home school as appropriate.
Other duties as assigned
CERTIFICATES, LICENSES, & REGISTRATIONS: Current Colorado Department of Education Teacher License. A Colorado teaching license with Special Education Generalist endorsement is preferred but the Special Education endorsement is not required.
EXPERIENCE: Preferred minimum 2 years successful teaching experience working with diverse student populations.

Ability to meet physical needs of students. Position requires normal auditory acuity and sound discrimination skills to teach phonetics in reading.
