Position Title: Care Partner
Department: Jennings at Notre Dame Village
Reports To: Executive Director
Positions available: Full Time 1st shift and 2nd shift hours available.
Position Summary:
Care Partners are responsible for the delivery of holistic resident centered, resident-directed care that promotes maximization of function, resident autonomy, and choice. The responsibilities of the Care Partner also include care of the elder and the residents’ environment, including cooking, laundry, and light housekeeping.
- Essential duties and responsibilities which must be performed in order to carry out the position purpose summarized above:
- Communicates a sense of caring, concern, and dignity for residents.
- Places decision-making in the hands of the residents whenever possible and appropriate.
- Makes prompt and accurate judgments about elder care and emergencies.
- Follows the service plan in all matters related to care and services.
- Recognizes the autonomy and dignity of all residents.
- Complies with all policies and procedures of the organization.
- Participates in the quality assurance program.
- Serves on committees and workgroups.
- Excellent attendance including reports to work on time.
- Participates and attends team meetings.
- Works collaboratively with all staff members in the house.
- Communicates with all in a respectful manner.
- Knows and follows all emergency procedures
- Follows safe chemical handling practices.
- Reports any safety concerns immediately.
- Listens to and gives reports.
- Provides care to residents by assisting with ADLs as needed.
- Provides personal care as outlined in the resident’s service plan. Personal care may include shower/bath, shampoo, perineal care, oral hygiene, positioning, nails short and clean, dressing, ambulation and insuring that adaptive devices are worn (i.e. dentures, hearing aids, glasses, etc.).
- Provides medication reminders and assistance with self-administration in accordance with policy.
- Observes residents and reports any reactions or changes in condition to the nurse.
- Conducts menu planning meetings with residents using menu planning software.
- Prepares and serves meals, and snacks between meals, bedtime and other nutritional snacks per the stated preferences of the elder.
- Cleans up after meals and snacks.
- Launders resident’s personal clothing, bed linens and towels.
- Cleans and maintains the environment of the home and follows housekeeping schedules.
- Documents care as required in the electronic medical record.
- Performs other related duties as required by the elder.
- Identifies and honors resident choices in activities of daily living while maintaining resident safety.
- Provides engagement and activities for residents.
- Reports accidents and incidents when they occur.
- Attends all required educational programs per administrative policies and procedures.
- Other duties and responsibilities:

- Observe all facility safety policies and procedures.
- Observe all infection control policies.
- Accept assigned duties in a cooperative manner.
- Assume accountability for information contained in the Staff Handbook.

Qualifications:
3A. Specific knowledge, training or skills required to perform the duties of this position. - ServSafe® certification as required by the State of Ohio prior to or upon hire
- C.P.R.
- First aid training.
- Dementia care training prior to or upon hire.
- Understanding of and experience in successfully engaging persons with dementia
- Computer skills–MS word, Internet, e-mail, electronic medical records.
- Cooking skills.
- Self-administration medication training prior to or upon hire
- Understands and communicates job-related information and directions in English
3B. 3C. Previous experience that is necessary background to qualify for this position:
- Experience providing personal care to the elderly, STNA or CNA status preferred
Qualifications
- C.P.R. training prior to or upon hire
