Position Summary:
(Overall purpose of this position)
The small house nurse provides person-centered, rehabilitative, holistic care and chronic disease management to residents in small houses. The nurse provides clinical leadership and assures that staff are providing care per the care plan.

- Interacts with all staff members in a manner that is respectful and collaborative.
- Mentors, nurtures and fosters growth in all clinical staff. Inspires high performance through words and actions. Provides tools and resources to empower staff members to deliver excellent holistic clinical care.
- Helps to ensure all clinical staff consistently incorporate resident autonomy, dignity, and choice as core values in their service delivery.
- Promotes services that support independence and holistic wellness.
- Helps to ensures that residents are involved in decision making
- Ensures that clinical decisions are made using the “at-home” principles.
- Participates in a collaborative quality assurance and compliance monitoring program.
- Establishes and maintains positive productive relationships with families and community members.
- Conducts regular clinical rounds to evaluate compliance with service plan, resident preferences and effectiveness of outcomes.
- Promotes, expects and role models an “all hands-on deck” approach to meeting
resident needs.
- Promotes standards and collaborates with the care partners in planning resident care.
- Conducts clinical rounds on every resident every day.
- Based on outcome of rounds adjusts care.
- Plans and provides feedback and guidance to care partners.
- Participates in house work, including cooking cleaning and laundry as needed to meet resident needs.
- Leads quality assurance and infection control in the houses.
- Provides care to meet resident needs including direct care, treatments, and oversight of self-administration of medications through medication reminders.
- Participates in the Clinical Team meetings.
- Documents care as required in the electronic medical record.
- Collaborates with the physician and pharmacist to manage resident medical needs and medications.
- Maintains electronic medical chart and responds to resident record requests.
- Schedules resident medical appointments in coordination with resident family members.
- Audits medications against physician orders, assesses their effectiveness and monitors for side effects.
- Monitors and oversees medication reminders performed by staff including medication reminder logs and audits.
- Provides treatments as ordered by the physician.
- Acts as a care manager - conducts assessments, leads service planning and manages family expectations for the residents.
- Contacts the primary family member as needed.
- Participates as needed in house resident listening sessions.
- Contributes to the community’s education and growth by providing teaching as needed.

Qualifications:

- Current State of Ohio license as a registered nurse or licensed practical nurse.
- Understanding of and experience with successfully interacting and supporting persons with dementia.
- Willing to promote standards of person-centered care
- Strong computer skills.
- Good organizational and multitasking ability.
- Critical thinking.

3C. Previous experience that is necessary background to qualify for this position:
- Experience in geriatric nursing and dementia
Qualifications
