Position Title: Neighborhood Coordinator
Department: Nursing
Reports To: Resident Care Coordinator – Brecksville
Hours: Part Time 2nd shift hours available, 2-3 days per week, 2:00pm - 10:30pm
Position Summary:

Provides nursing care and supervision of care partners to assure quality care and resident safety.

Essential Functions
- Follow established performance standards and implement facility policies and procedures on all Jennings at Brecksville resident neighborhoods.
- Interpret existing policies and procedures to care partners, residents, families and physicians.
- Supervise and evaluate all care and activities provided within the resident neighborhoods and initiate corrective action as appropriate. Communicates staff observations to Resident Care Coordinator.
- Obtain report from nurse on prior shift and record sufficient information to implement appropriate follow-up action as necessary in relation to the resident’s care needs.
- Conduct resident rounds as assigned to assess the condition of each resident and report problems to Resident Care Coordinator and initiate necessary follow-up action.
- Identify safety hazards and initiate corrective action.
- Identify special nursing problems and emergency situations quickly and initiate immediate “life-saving” measures in the absence of a physician.
- Follow and supervise the implementation of the “Resident Bill of Rights”.
- Provide clinical supervision to care partners.
- Attend staff meetings and in-service classes regularly.
- Instruct care partners in proper, preventive safety measures and use of adaptive equipment to meet the needs of each neighborhood.
- Control supplies and equipment within the resident neighborhoods.
- Assume responsibility for compliance with federal, state and local regulations within the resident neighborhoods on the assigned shift of duty.
- Counsel supervised staff and recommend disciplinary action to the Resident Care Coordinator.
- Administer medications and treatments per physician’s order and accurately record medications and treatments as administered.
- Be responsible for the safety of residents under his/her supervision.
- Document resident care provided and resident’s response or lack of response to treatment provided following proper documentation procedures; notes are complete, accurate, and written legibly and/or recorded accurately in the EMR; charts weekly wound descriptions, incidental notes, monthly summaries, 24-hour notes.
- Assess and report changes in resident’s condition including development of decubitus to supervisor and take follow-up action as necessary.
- Assess on a weekly basis the skin condition, stage, size (measurements), sites, depth, color, drainage and odor of condition.
- Assist in the admission/transfer and discharge of residents as necessary.
- Complete admission/transfer and discharge documentation. Update information as transfers, discharge, leave of absence etc. occur.
- Perform basic nursing skills.
- Obtain physician’s orders and transcribe orders per facility policy. Schedule x-rays and labs and enter labs into the labs system.
- Assess resident needs and communicate information to the Resident Care Coordinator in order to develop an individualized care plan.
- Participate in resident care conferences either within written information or by attending the conference.
- Provide direct resident care.
- Consistently work cooperatively with residents, all nursing personnel, physicians, families, consultant personnel, community agencies and ancillary personnel.
- Render immediate assistance to any resident in need.
- Supervise the answering of call lights within unit assigned.
- Listen to resident, family or physician complaints and initiate corrective action and/or report special problems to the supervisor.
- Obtain continuing education related to the long term care field of nursing and as required by the Ohio Board of Nursing.
- Administer tube feedings.
- Administer oral and parenteral medications according to facility policy. Recognize drug actions and interactions, and side effects.
- Perform blood glucose check and administer insulin.
- Monitor residents’ dietary intake by observation, and by checking the daily intake record.
- Facilitate the distribution and feeding of daily nourishments.
- Check and re-order medications; maintains and accurate control drug count.
- Review and monitor vital signs, and weights taken by nursing assistants. Monitors BM flow sheet and administers enemas, laxatives when appropriate.
- Obtain urine, wound, and sputum specimens.
- Update ADL Directives.
- Come to work as scheduled and consistently demonstrate dependability and punctuality.
- Come to work in a clean, neat uniform and consistently present an appropriate professional appearance.
- Monitor Intravenous Fluid administration within the parameters of the Ohio Board of Nursing.
- Complete incident reports and investigation by end of the shift on which the incident occurs. Reports all incidents to Supervisor; notifies family or responsible party for change in condition.
- Attend monthly Resident Neighborhood meetings.

Qualifications:

3C. Previous experience that is necessary background to qualify for this position:
Experience in long term care preferred.
Qualifications
