Position Title:   Neighborhood Coordinator

Department:      Nursing

Reports To:       Resident Care Coordinator – Brecksville

Hours: Part Time 2nd shift hours available, 2-3 days per week, 2:00pm - 10:30pm

Position Summary:

          (Overall purpose of this position)

Provides nursing care and supervision of care partners to assure quality care and resident safety.

Essential Functions

  1.  Follow established performance standards and implement facility policies and procedures on all Jennings at Brecksville resident neighborhoods.
  2.  Interpret existing policies and procedures to care partners, residents, families and physicians.
  3.  Supervise and evaluate all care and activities provided within the resident neighborhoods and initiate corrective action as appropriate.  Communicates staff observations to Resident Care Coordinator.
  4.  Obtain report from nurse on prior shift and record sufficient information to implement appropriate follow-up action as necessary in relation to the resident’s care needs.
  5.  Conduct resident rounds as assigned to assess the condition of each resident and report problems to Resident Care Coordinator and initiate necessary follow-up action.
  6.  Identify safety hazards and initiate corrective action.
  7.  Identify special nursing problems and emergency situations quickly and initiate immediate “life-saving” measures in the absence of a physician.
  8.  Follow and supervise the implementation of the “Resident Bill of Rights”.
  9.  Provide clinical supervision to care partners.
  10.  Attend staff meetings and in-service classes regularly.
  11. Instruct care partners in proper, preventive safety measures and use of adaptive equipment to meet the needs of each neighborhood.
  12. Control supplies and equipment within the resident neighborhoods. 
  13. Assume responsibility for compliance with federal, state and local regulations within the resident neighborhoods on the assigned shift of duty.
  14. Counsel supervised staff and recommend disciplinary action to the Resident Care Coordinator.
  15. Administer medications and treatments per physician’s order and accurately record medications and treatments as administered.
  16. Be responsible for the safety of residents under his/her supervision.
  17. Document resident care provided and resident’s response or lack of response to treatment provided following proper documentation procedures; notes are complete, accurate, and written legibly and/or recorded accurately in the EMR; charts weekly wound descriptions, incidental notes, monthly summaries, 24-hour notes.
  18.  Assess and report changes in resident’s condition including development of decubitus to supervisor and take follow-up action as necessary.
  19.  Assess on a weekly basis the skin condition, stage, size (measurements), sites, depth, color, drainage and odor of condition.
  20.  Assist in the admission/transfer and discharge of residents as necessary. 
  21.  Complete admission/transfer and discharge documentation.  Update information as transfers, discharge, leave of absence etc. occur.
  22.  Perform basic nursing skills.
  23.  Obtain physician’s orders and transcribe orders per facility policy.  Schedule x-rays and labs and enter labs into the labs system.
  24.  Assess resident needs and communicate information to the Resident Care Coordinator in order to develop an individualized care plan.
  25.  Participate in resident care conferences either within written information or by attending the conference.
  26.  Provide direct resident care.
  27.  Consistently work cooperatively with residents, all nursing personnel, physicians, families, consultant personnel, community agencies and ancillary personnel.
  28.  Render immediate assistance to any resident in need.
  29.  Supervise the answering of call lights within unit assigned.
  30. Listen to resident, family or physician complaints and initiate corrective action and/or report special problems to the supervisor.
  31.  Obtain continuing education related to the long term care field of nursing and as required by the Ohio Board of Nursing.
  32.  Administer tube feedings.
  33.  Administer oral and parenteral medications according to facility policy.   Recognize drug actions and interactions, and side effects.
  34.  Perform blood glucose check and administer insulin.
  35. Monitor residents’ dietary intake by observation, and by checking the daily intake record.
  36.  Facilitate the distribution and feeding of daily nourishments.
  37.  Check and re-order medications; maintains and accurate control drug count.
  38.  Review and monitor vital signs, and weights taken by nursing assistants.  Monitors BM flow sheet and administers enemas, laxatives when appropriate.
  39.  Obtain urine, wound, and sputum specimens.
  40.  Update ADL Directives.
  41.  Come to work as scheduled and consistently demonstrate dependability and punctuality.
  42.  Come to work in a clean, neat uniform and consistently present an appropriate professional appearance.
  43.  Monitor Intravenous Fluid administration within the parameters of the Ohio Board of Nursing.
  44.  Complete incident reports and investigation by end of the shift on which the incident occurs.  Reports all incidents to Supervisor; notifies family or responsible party for change in condition.
  45.  Attend monthly Resident Neighborhood meetings.

  5. Follow and assume accountability for all policies and procedures contained in the Staff Handbook.

  7. Support campus wide activities by offering ideas, participating on committees and attending staff and resident events.
  8. Function as an active member of the team, supporting fellow team members and working effectively with other individuals and teams.

  11. Perform all other related duties as directed by your supervisor and/or manager.

3C.     Previous experience that is necessary background to qualify for this position:

Qualifications
