Duties and Responsibilities:
- Provide psychiatric evaluations and medication management for geriatric clients in an outpatient clinic setting, at a clinic location, via telehealth, or in the home, if needed
- To participate with other staff members in the development of a treatment plan based on the diagnostic assessment, to periodically reassess patients’ needs and circumstances and revise treatment plan if appropriate;
- To help coordinate the patient’s care with hospitals and other providers;
- To help with hospitalization when appropriate;
- To conduct seminars and play a part in the education of the team where time permits and the necessity arises;
- To be available to the team for consultations, when necessary.
- Attends and participates in staff meetings and periodic in-service trainings when available.
- Additional tasks as assigned.
Requirements:
- Valid NYS license to practice in discipline
- Current DEA Registration Number
- Possession of or ability to obtain a NPI number and Medicare Provider Number
- Desire and ability to work collaboratively and engage in quality improvement activities as part of a committed, integrated team
- Excellent interpersonal skills and the ability to communicate effectively and concisely both in writing and orally
- Strong attention to detail with the ability to solve problems; to elicit, comprehend, and synthesize accurate and specific information; prioritize work; to work independently; and to excel in a high-performance culture

- Good writing/record keeping skills
- Basic computer skills
- Familiarity with electronic health records and e-prescribing desired
- Ability to read, write speak and understand English
