What are the responsibilities and job description for the Senior Case Manager position at Jewish Community Council of Greater Coney Island?

Coney Island Based DOP Works Plus Program is seeking a motivated and passionate full time Senior Case Manager. The DOP Works Plus Program provides work readiness and wrap-around services for teens and young adults in the community. Program participants are referred to the program through New York City’s Crisis Management Systems initiative (CMS). Works Plus offers job readiness training, skills workshops and job placement assistance, aiming to help participants secure stable employment.  The program also connects individuals with educational opportunities and vocational training to enhance long-term career prospects. Responsibilities include:

  • Providing services to 17 participants annually for up to 6 months of work readiness and up to 6 months of follow up/ supportive services.
  • Conducting a baseline assessment of each participant to assess individual needs and level of work readiness
  • Tracking participant data and outcomes
  • Making program improvements based on the review of programmatic data
  • Maintaining relationships with the referring CMS provider to ensure sufficient and appropriate referrals to the program
  • Structuring the program operation hours to make participation ad widely accessible as possible, offering both day and evening programming on different days of the week
  • Providing services to meet participants “where they are at” and develop an individualized program plan which guides the participants through their program participation
  • Ensuring each participant completed 4/8 core activities which can be provided individually or in group settings as needed
  • Leading workshops and trainings to promote soft skills development

 

Qualifications:

  • Bachelors degree in Human Services or related field preferred
  • 2-3 years of experience in case management, workforce development or a related field, ideally in the non-profit or social services setting.
  • Knowledge of career development practices, job training programs and soft skills development
  • Ability to work empathetically with diverse populations including individuals facing unemployment, underemployment or other socio-economic barriers.
  • Ability to assess participants needs, develop individualized program plans and advocate for necessary services and resources
  • Ability to manage a caseload, prioritize tasks and meet deadlines in a fast-paced environment.
  • Familiarity with community resources including local job markets and workforce development resources to connect participants to appropriate services
  • A passion for youth and community services.
  • A resident of the Coney Island Community is a plus!

 

On-Site. 30 hours per week. Late afternoons-evening. Monday through Friday (specific hours are somewhat flexible and related to scheduled events and program needs).

Pay range:  50-55k annually
