What are the responsibilities and job description for the Social Worker position at Jewish Community Council of Greater Coney Island?
JCCGCI is the largest provider of Holocaust Support Services outside of Israel and Russia. We are seeking a Social Worker to join our passionate and dedicated Holocaust Support Services team to provide in-depth eligibility assessments and case management for homebound Holocaust survivors throughout Brooklyn.
Responsibilities include:
- Conducting in-home assessments and reassessments.
- Identifying unmet needs of each homecare client.
- Making every effort to assure that the unmet needs of each client are responded to through information, referrals, advocacy and/or direct intervention by the social worker.
- Interacting with each client’s family as needed to address issues affecting the health and welfare and quality of life of the client.
- Scheduling home visits to clients in as efficient manner as possible to enable as many clients as possible to be assessed and reassessed.
- Recording of relevant information about clients and assessment and reassessment outcomes in management information system.
Requirements:
- Bachelor’s degree in Social Work or related field required. MSW Preferred.
- Friendly disposition and caring attitude toward seniors.
- Strong problem solving and critical thinking abilities.
- Effective communication skills.
- Computer literacy a must.
- Bilingual Yiddish or Russian a plus.
- Travel to clients’ homes required.
Full time 35 hours per week, on site in office and in the field visiting client homes.
Annual Salary: $55,000-60,000.
