Job Overview

We are seeking a dedicated and compassionate Social Worker to join our team. The ideal candidate will have experience in an integrated team approach in providing in home care management services and supportive counseling for Hoarding Support Program clients, ages 18 . The SW/CM identifies the needs of clients and provides hoarding case management services and facilitates Buried in Treasures support groups. This position provides in-home, direct services for clients who are experiencing hoarding behaviors. Services may also be provided in the Northeast HUB (northeast), OIC Philadelphia (north), Brodsky Center (Bala Cynwyd) offices and in-home. Groups may be provided via telehealth or in person. The Social Worker will play a crucial role in supporting clients' needs while ensuring compliance with HIPAA regulations. Travel is a must.

Duties and Responsibilities

  • Coordinates and provides supportive counseling and care management services for clients with hoarding disorder in the client’s home. There is a large proportion of time spent in clients’ homes for this duty. The average caseload of individuals clients will be between 14-20 depending on geographic area, we serve the 5-county area.
  • Conducts comprehensive assessment of clients’ needs and assesses their goals to develop mutually agreed upon treatment/care plans.
  • Works with clients to enable access to resources appropriate to their individual needs (e.g., referrals to legal services, mental healthcare, and increasing supports in the home).
  • Collaborates with community resources and assists clients with daily life skills to help them function at as high a level as possible.
  • Maintains knowledge of a broad variety of community resources and refers clients when appropriate to support their treatment goals.
  • Provides Motivational Interviewing and Cognitive Behavioral Therapy skills to clients with hoarding behaviors including direct support with sorting and discarding. Utilize the program model to ensure the client’s home becomes safe and sanitary, spaces can be used for intended purpose.
  • Provide in-home support to all clients with hoarding behavior and/or hoarding disorder.
  • Provide advocacy with landlords, community enforcement agencies, family and natural supports, and others as needed.
  • Provide psychoeducation to clients, family and natural supports, and community resources regarding hoarding disorder and other mental health needs.
  • Prepares and documents accurate and timely records including clinical and progress notes, assessment tools, and billing information in compliance with department documentation guidelines and expectations and external regulatory policies.
  • Participate in team conferences, peer supervisory sessions, weekly supervision, department meetings, and other meetings related to care management issues.
  • Facilitates Buried in Treasures, and other related support groups, ongoing and as needed. Groups may take place during the day or evening based on client need. Training will be provided. The expectation will be that at least one cohort is running at a time, if need and capacity allow, we may increase.
  • Assist in facilitation of training on Hoarding Disorder with support from team. Training will be provided.
  • Other duties as assigned by direct supervisor.

  • Demonstrates initiative in contributing to the development of the department’s programs and services as well as his/her own professional development.
  • Must be in good standing, maintain any pertinent license(s), and follow guidelines as required by the PA State Board of Social Workers, Marriage and Family Counselors, and Professional Counselors.
  • Demonstrate knowledge, understanding, and implementation of practices based on the NASW code of ethics.
  • Participates in annual Trauma Informed Care training(s).
Requirements:


Required Qualifications

  • MSW preferred, or 5 years of experience, preference may be given if experience with those who have Hoarding Disorder.
  • Two to three years of direct service experience required
  • Proficient user of technology (smart phone/tablet/laptop) and experience using MS Office applications required
  • EHR/EMR database experience a plus

Additional Requirements

  • Work hours may include evening and weekends.
  • Employee must have a car. Travel is required.
  • Employee must be comfortable going into various neighborhoods throughout the 5-county area (Philadelphia, Chester, Montgomery, Bucks, Delaware) and home visits.
  • Employee must have a safe driving record on file with the Department of Motor Vehicles in the state in which the employee is licensed to drive. Employee must provide a valid driver’s license and proof of automobile liability insurance. In addition, employee must provide security clearances free of any citations.
  • Employee must pass all necessary criminal and/or background checks.
  • Employee must meet JFCS vaccination requirements at hire and as required by policy.
