Company Overview
The Philadelphia Model: Addressing Serious Mental Illness (SMI) in Long-Term Care (LTC) will provide highly structured therapeutic residential behavioral healthcare and treatment for frail adults who are eligible for skilled nursing level of care who also have serious mental illness and who have reached maximum benefit from the mental health resources available elsewhere in the community or hospital. The program philosophy is to create a welcoming, safe, and supportive environment where frail older adults with SMI that may include other complex behavioral health needs and/or prior criminal history – and need long-term skilled nursing services – can receive the care and services needed to address their holistic needs free of stigma. A multi-disciplinary team will provide both long-term care and behavioral health services in this integrated program. Job Summary
The Social Worker provides clinical social work services to patients facing mental health challenges, substance use disorders, and co-occurring conditions. Responsibilities include case management, discharge planning, resource coordination, counseling, and crisis intervention while working closely with patients, families, and interdisciplinary treatment teams.
Key Responsibilities
- Complete psychosocial assessments, mental health evaluations, and risk assessments
- Develop individualized treatment and discharge plans in collaboration with the clinical team
- Provide counseling, support, and advocacy for patients and families
- Facilitate referrals to community resources, including housing, vocational programs, healthcare, and addiction recovery services
- Participate in treatment team meetings, case reviews, and interdisciplinary care planning
- Maintain accurate documentation in the Electronic Health Records (EHR) system
- Ensure compliance with state, federal, and hospital policies
- Conduct crisis intervention and suicide risk assessments as needed
Qualifications
Education:
- Bachelor’s or master’s degree in social work (BSW/MSW), Psychology, Human Services, or a related field
Experience:
- Minimum 1 year of experience in mental health, substance abuse, or behavioral health case management
- Experience in inpatient psychiatric care or addiction recovery programs is a plus
Licensure:
- LSW or LCSW preferred
- CPR Certification preferred
Skills:
- Knowledge of trauma-informed care, harm reduction, CBT, and Motivational Interviewing
- Strong interpersonal, communication, and clinical documentation skills
- Proficient in Electronic Health Records (EHR), Microsoft Office, and community resource navigation
- Ability to work both independently and as part of a collaborative multidisciplinary team
Work Environment & Physical Demands
- Interact directly with patients in inpatient psychiatric units, substance abuse programs, and outpatient settings
- Ability to manage emotionally intense and high-stress situations, including clients in crisis
- Flexibility to work occasional weekends or holidays as needed
