Responsibilities
- Provides home- or community-based therapeutic and supportive services for children with serious behavioral-health concerns and their families.
- Assists children and families in obtaining community support, identifying resources, and resolving barriers to overall life success.
- Serves as active participator in Child and Family Team (CFT) meetings and completes all related documentation in a timely manner.
- Performs necessary tasks to help families understand the CFT process and remain involved in decision-making.
- Supports the most appropriate and least restrictive levels of care for involved youth.
- Works collaboratively with medical, treatment, and management staff to ensure best practices of care for children receiving services.
- Bachelor's Degree in a human services field preferred; a combination of relevant experience and education may be considered.
- Experience working with youth in a behavioral-health setting.
- Level 1 Fingerprint Clearance Card
- Acceptable Driving Record
- Ability to engage and work with diverse cultural groups.
- Bilingual, Bicultural a plus.
