- Job Overview:
As a Food Security Case Manager, you will play a crucial role in addressing food insecurity for households in the community by providing direct support and resources to individuals and families in need. This position involves assessing client needs, developing personalized service plans; and connecting clients to necessary resources; including food assistance programs such as SNAP, nutrition education and other supportive services.
Responsibilities:
- Conduct assessments of clients’ food security status and related needs.
- Develop individualized service plans based on clients’ needs and available resources.
- Gather information through face to face engagement, questionnaires, and home visits to identify barriers to food access.
- Connect clients to local food banks, soup kitchens, nutrition programs, and other community resources.
- Maintain up to date knowledge of available food assistance programs and eligibility requirements.
- Engage in outreach events to raise awareness of food insecurity.
- Collaborate with local organizations, government agencies, and community stakeholders to strengthen food insecurity initiatives.
- Transport clients to local grocery stores and assist with one-on one support while in the stores.
- Maintain accurate and confidential client records in compliance with agency policies.
- Participate in staff meetings and training sessions to enhance service delivery.
Position Requirements
- Associates degree required; Bachelor’s Degree in Social Work, or related field preferred.
- - Position requires ability to drive mobile food truck (CDL not required) and other agency vehicles. Must possess valid NJ Driver’s license. When using agency or own vehicle, employee must have a Valid NJ driver’s license and proof of NJ Auto Insurance
- – This full-time position requires flexibility. Current schedule is Monday through Friday 9-5pm; however, the role will require some evenings and weekends.
