This position is responsible for administering, maintaining, and expanding a program of primary (and in selected instances, secondary care) and preventive medical care in a community health center setting.
The Family Medicine Nurse Practitioner, an experienced ARNP, will act within their professional boundaries to provide care for patients with initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care plans. The provider will demonstrate safe, clinical decision-making and expert care for patients within the practice. The provider will work collaboratively with a multi-disciplinary team to meet the needs of patients, supporting the delivery of care according to JFS policy and procedures. The provider will build effective working relationships with other health care professionals. The provider will work with a team of clinical support staff to deliver excellent primary health care services within the scope of a medical home model.
ESSENTIAL DUTIES and RESPONSIBILITEIS
Management of Patient Health
- Deliver a high standard of patient care as a Family Medicine Nurse Practitioner, using advanced clinical skills, and a broad and in-depth theoretical knowledge base
- Provide a first point of contact within the practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan
- Prioritize health problems and intervene appropriately, including initiation of effective emergency care
- Initiate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports
- Prescribe safe, effective, and appropriate medication using evidence-based guidelines and recommendations
- Provide safe, evidence-based, cost-effective, individualized patient care
- Promote health and well-being using health promotion, health education, screening and therapeutic communication skills
- Encourage and support patients’ ability to self-manage chronic conditions and to engage in preventive health measures
- Communicate the health status of the patient using appropriate terminology, technology, and format
- Work within the practice guidelines, policies, and protocols
- Maintain complete, accurate and timely medical records, utilizing electronic health record software.

- Mitigate risk within the practice by working to prevent sentinel events using proper infection control and patient safety strategies
- Participate in the analysis of patient satisfaction data, as a means of quality improvement within the practice
- Participate, actively, in the delivery of productivity and revenue targets as set by JFS’ chief officers.

- The Nurse Practitioner must comply with all JFS health and safety policies, by following safe working procedures and following incident reporting protocols. Specifically, they must:

- Refrain from the willful misuse of the position or interference with protocols that are developed in the interest of health and safety.
- Report any hazards and defects present in the clinical environment.
- Report any accidents or incidents in the clinical environment, completing all appropriate forms as soon as possible.
- The Nurse Practitioner is required to comply with the rules, regulations, standing orders and financial instructions set forth by JFS policies.
Qualifications
- Graduate of an accredited nurse practitioner program
- Nurse Practitioner Certification in Family Medicine
- Current Texas Medical License
- Current DEA License
- Current certification in Basic Life Support (BLS)
- At least 1-2 years of experience as a nurse practitioner in an outpatient setting is required.
