The Community Health Navigator will play a pivotal role in fostering connections and collaboration between JFSWM and the Refugee community. This position involves developing and implementing strategies to engage community members, addressing their needs, and promoting social integration.
1. Health Case Management and Navigational Support
a. Conduct thorough assessments to identify health-related needs and concerns within the community.
b. Complete necessary follow-up activities, supporting clients to schedule appointments and ensuring they get the appropriate care needed
c. Assist refugees in understanding medical terminology, navigating the healthcare system, interpreting health-related documents, and advocating for their health needs
d. Complete all necessary documentation of work with clients, including case notes and support with bi-annual reporting
2. Community Health Outreach
a. Cultivate and maintain positive relationships with members of the refugee community, local organizations, and stakeholders to strengthen ties and collaboration.
b. Provide guidance on cultural nuances to enhance effective engagement and communication.
c. Explore opportunities for joint initiatives and resource-sharing to benefit the Refugee community.
d. Collaborate with healthcare providers and community organizations by establishing and maintaining partnerships with healthcare providers, local clinics, community organizations, and social service agencies to coordinate and enhance the delivery of health education programs.
3. Education and Group Programming
a. Conduct outreach activities to raise awareness about preventive health measures and available healthcare resources to JFSWM clients.
b. Provide education and training sessions on health-related topics to community members
c. Lead group meetings to provide ongoing support and education on health and cultural related topics to community members.
:
· Bachelor's degree in social work, public health, human services, or a related field. Experience working with refugee populations is highly desirable.
· Knowledge of refugee health issues, cultural sensitivity, and experience working within diverse communities.
· Knowledge of local resources, services, and agencies in Western Massachusetts, specifically those serving refugee populations, is preferred.
· Fluency in English and another relevant language is required (Haitian Creole or Spanish preferred)
· Previous experience in community engagement, preferably within Refugee Communities or similar cultural contexts.
· Demonstrated ability to plan and organize community events and initiatives.
· A valid driver’s license with regular access to a reliable vehicle is required.
Job Type: Full-time
Pay: $20.00 - $22.00 per hour
Expected hours: 37.5 per week
