Part Time 20 hours Days
GENERAL SCOPE: The Dementia Care Navigator provides compassionate guidance and expert support to
individuals living with dementia and their families. Utilizing their expertise, the navigator helps families,
residents and caregivers access resources, provide education and navigate the complexities of dementia care.
This role involves advocacy, education, and coordination of services to ensure holistic, person-centered care
requiring the Navigator to have a flexible schedule to work occasional evenings and weekend hours.
EDUCATION / QUALIFICATION: Two or four-year degree in Human Services or a related field.
Minimum 2 years of experience in dementia care, senior care, healthcare, or social services. Dementia care
certifications (e.g., CDS - Certified Dementia Specialist) preferred.

EXPERIENCE: Strong knowledge of dementia-related diseases, caregiving challenges, and care planning
strategies. Excellent communication, advocacy, and crisis intervention skills. Ability to navigate complex

- Act as an advocate for patients and families within healthcare systems.
