In this role, you have the opportunity to use your organizational skills and public health experience to make a meaningful difference in your community and help change lives across San Diego. The Health Navigation Aide plays an integral role on the team and closely works with the Health Navigator. They support the daily work of the department’s Health Navigation programs by coordinating ongoing health follow-ups for newly arrived refugees, transporting clients, setting follow-up appointments, troubleshooting with healthcare providers, assisting with workshops, and bolstering partnerships between health clinics and JFSSD. This position reports directly to the Preferred Communities Program Manager and works closely with the Preferred Communities team and the rest of the Refugee Resettlement team.Responsibilities : Coordinate refugee health screenings for all new arrivals through the Refugee Health Assessment Program (RHAP)Coordinate client’s transportation to scheduled medical appointments, including urgent / emergency or specialty medical care health servicesAssess and coordinate referrals to mental health providers as neededAssist patients to enroll in a Medi-Cal managed care plan and select a primary care physicianProvide assistance with telehealth appointments through education and / or access to appropriate technologyConduct community outreach and presentations to develop a system of support on behalf of eligible populationsParticipate in collaborative and work group meetings related to health care servicesManage data and documentation per program guidelinesSkills / Experience / Abilities That Are a Must-Have : Fluency in Haitian-CreoleFamiliarity with the US healthcare system and healthcare programs for low-income families are strongly preferred2 years of experience with health education or working in the medical / health related fieldExperience working with refugee and / or immigrant populations strongly preferredSelf-starter with excellent problem-solving skills, proven ability to multi-task, prioritize duties, and manage time effectivelyStrong relationship building, diplomacy, and networking skills; ability to effectively build internal and external relationshipsDemonstrated success working and communicating effectively in a multi-cultural environmentExperience in a nonprofit setting, preferredAdvanced use of MS Office programs, including Outlook, Excel, and WordAbility to manage multiple priorities in a fast-paced environmentExcellent communication skills and customer relationship skillsSkills / Abilities We’d Like You to Have : Fluency in second language : Spanish or FrenchIncomplete submissions will not be considered.
