**Key Responsibilities:**
- Provide comprehensive primary care services to adult patients with a focus on preventative care, diagnosis, and treatment of acute and chronic illnesses.
- Conduct health risk assessments, develop personalized care plans, and educate patients on healthy lifestyle choices.
- Document patient encounters accurately and thoroughly using ICD-10 coding and Hierarchical Condition Categories (HCC).
- Maintain up-to-date knowledge of evidence-based practices, clinical guidelines, and medication management.

- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate required.
- Experience in primary care settings, including internal medicine, geriatrics, or family medicine.
- Strong understanding of Health Risk Assessments (HRAs), ICD-10 coding, and HCC documentation.
- Ability to work effectively in a team environment and communicate with patients, families, and healthcare professionals.
