Responsibilities
As a Permanent Social Worker, you will be responsible for
- Providing emotional and psychosocial support to patients and their families, particularly those dealing with terminal illness and end-of-life care situations.
- Conducting comprehensive psychosocial assessments and developing individualized care plans based on these assessments.
- Collaborating with a multidisciplinary team of healthcare professionals to ensure the holistic care of patients.
- Navigating complex medical information and explaining it in a way that patients and their families can understand.
- Advocating for patients' rights and needs, ensuring they are being met throughout their medical journey.
- Providing crisis intervention as needed and facilitating access to appropriate mental health resources.
- Assisting patients and their families in understanding and making decisions regarding treatment options, advance directives, and other end-of-life decisions.
- Coordinating with community resources to ensure continuity of care post-discharge.
The ideal candidate for the Permanent Social Worker position will possess the following qualifications
- A Master’s degree in Social Work from an accredited institution.
- A minimum of 5 years of experience as a social worker, preferably in a medical or psychiatric setting.
- Experience working with patients dealing with terminal illnesses and end-of-life care situations.
- A deep understanding of psychiatric principles, including the ability to conduct comprehensive psychosocial assessments.
- Exceptional communication skills, with the ability to explain complex medical information in a way that patients and their families can understand.
- Strong advocacy skills, with a proven track record of advocating for patients' rights and needs.
- The ability to work effectively as part of a multidisciplinary healthcare team.
- A compassionate, patient-centered approach to care, with a genuine desire to support patients and their families during challenging times.
