The Johnston County Health Department has a vacancy for a Public Health Nurse III/CMHRP Care Manager.
EXAMPLE OF DUTIES
Position functions as CMHRP Care Manager, enrolling pregnant women for Pregnancy Care Management. This will involve a case management approach for pregnant women who are identified as being at risk for poor birth outcomes with a primary focus on the organization of services and resources to respond to the health care needs of identified pregnant women throughout Johnston County. Employee will work with medical providers throughout Johnston County, marketing the program to ensure all eligible women have the opportunity to be enrolled. Experience in public health obstetrical care preferred.
SUPPLEMENTAL INFORMATION
Minimum Education and Experience for Program Manager, Clinic Coordinator, or Home Health Role - Graduation from a four-year college or university with a B.S. in Nursing which includes a Public Health Nursing rotation and two years of Public Health nursing experience; or a Master’s in Public Health and graduation from a school of professional nursing and two years of professional nursing experience; or graduation from a school of professional nursing and three years of professional nursing experience including two years of Public Health nursing experience; or an equivalent combination of training and experience.Necessary Special Qualifications – A current license to practice as a Registered Nurse in North Carolina by the N.
