Position Overview:
Case Manager is a full-time entry-level position that collaborates with clients to meet their needs. A Case Manager must be energized by talking with people and listening carefully to a person’s life story; and must be flexible in how they solve problems. The Case Manager will carry a caseload of longer-term clients to help these clients meet short-term and long-term goals focused on personal stability. In addition, the Case Manager will perform intakes with various clients as directed by management. Experience in human services is a plus, but not necessary. The right candidate can easily learn the skills to be an excellent human services worker.
Key Responsibilities:
DIRECT CLIENT SERVICES | 90%
- Conduct individual sessions with clients focused on goal planning, collaborative problem solving, and linkage and referrals to other services like job, housing, and mental health services.
- Coordinate and participate in the case review process.
- Document all services and interactions thoroughly and promptly.
- Collaborate closely with the staff team on case reviews and care coordination.
- Perform other duties as assigned by the Clinical Director.
NETWORKING | 5%
- Interface with other service agencies in the area to ensure seamless client service referral.
- Attending workshops and training to ensure best practices.
ADMINISTRATIVE FUNCTIONS | 5%
- Research community resources and update agency resource guides on a consistent basis to ensure quality referrals.
A Case Manager must be willing to:
- Learn about the experiences of people with housing instability,
- Empathize with the emotional experiences of people challenged by housing instability.
- Problem solve in a collaborative manner with people.
- Work with multicultural communities.
- Become more culturally competent over time.
- Establish and maintain appropriate boundaries with staff, volunteers, and all stakeholders.
- Seek supervisory help regularly and accept constructive criticism to become more skilled.
- Build rapport quickly with people from a wide variety of cultural and socioeconomic groups.
- Live out the agency’s core values of Respect, Teamwork, Mission Driven Work, and Commitment while on the job.
- Use Microsoft products and a web-based database or be willing to be trained on them.
- Demonstrate a passion to help the underserved.
- Learn about the communities and non-profit organizations within JOURNEYS service area.
- Collaborate with a tight knit clinical staff team.

The work schedule varies depending on the needs of the agency. Occasional evening and weekend work is necessary in order to attend meetings or to participate in events. This position is primarily sedentary, may require exerting up to 20 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects.
